NCISAA and CDS Required Athletic Forms:

As an NCISAA and CDS requirement for athletic participation, all 5th–12th graders interested in participating in a CDS sports team (not a clinic) are required to submit a 22-23 Sports Physical through the Magnus Health Portal prior to participating in preseason activity. A link to the Magnus Health Portal can be found in My CDS under Resources. Please use the “student health tracker” tab for downloading and uploading the required forms. CDS does not accept original paper documentation.

All athletic forms are due prior to the start of pre-season practices.

Sports Medicine and Athletic Training:

Our sports medicine department exists to ensure our student-athletes receive quality medical care while participating in athletics and physical education classes. We have two Athletic Training Rooms where athletes can receive health care services. They are located in the Nash Athletic Center and at our off-campus Sgro Athletics Complex.  Our full-time NATA (National Athletic Training Association) certified Athletic Trainer, Amanda Matos, works in conjunction with Blue Ridge Bone and Joint/EmergeOrtho orthopedic physicians and sports medicine staff to prevent, diagnose, and treat athletic injuries.  Carolina Day also employs a part time Athletic Trainer through PT Solutions who covers all after-school athletic practices and games. Our team doctor, Dr. Judd Garbarino, works closely with the athletic trainers during the school year to coordinate office visits, on-campus injury clinics, full rehab programs, and concussion treatment and care. Additionally, all Carolina Day School coaches are trained in concussion protocol and are CPR certified to help in the event of an emergency. Our goal is to provide accurate and timely injury prevention, evaluation, treatment, rehabilitation, return-to-play protocol, and education to our athletes.

Athletic Training Room Hours

Monday- Friday 10:00am to 6:00pm (Closed on No school days)

Upper School Sports Medicine Internships: In 2020, the Sports Medicine Department began accepting interested 9th–12th graders as Sports Med Interns to provide interested students with a hands-on Athletic Training experience. Students selected for the seasonal internships work alongside the Athletic Trainers to learn basic first aid and sports medicine skills that they practice with CDS athletes. The program teaches basic first aid, sports taping techniques, physical therapy and rehabilitation exercises, and concepts of orthopedic anatomy and physiology. In addition to collecting volunteer hours, the students selected for the internship also have the opportunity to become First Aid/AED and CPR certified. Students who excel in the program may have the opportunity to further their experience by shadowing a team doctor from Emerge Ortho over the summer. For more information regarding the Sports Management Program, please contact Director of Sports Medicine Amanda Matos at amatos@carolinaday.org.

Concussion Management:

In 2021, Carolina Day adapted a new form of Concussion testing called Sway Medical. All student-athletes undergo a yearly cognitive baseline assessment prior to the start of any contact sport.

Sway is a user-friendly, mobile device-based testing program specifically designed for the management of sports-related concussions. The Sway System is a research-backed assessment tool that includes a battery of balance tests, cognitive tests, symptom tracking, on-field assessments, and COVID-19 screening. The system is a clinically validated objective assessment for baseline testing, sideline assessment, and post-injury evaluation.

To increase and ensure the safety of our student-athletes, Carolina Day follows the NC Gfeller-Waller Concussion Law. The Gfeller-Waller Concussion Awareness Act was drafted and implemented to protect the safety of student-athletes in North Carolina and was signed into law on June 16, 2011 by Governor Beverly Purdue. There are three major areas of focus in the law: education; emergency action and postconcussion protocol implementation; and clearance/return to play or practice following concussion.

CDS also has in place a concussion management team (which consists of our school nurse, Athletic Trainer, coaches, student advisors, and team doctor) that ensures a smooth and consistent return-to-play process after a concussion has been diagnosed. Once a concussion has been diagnosed, the concussion management team ensures that the athlete completes both phases of the GFW concussion protocol, which includes a return-to-learn phase followed by a gradual 5-step return-to-play phase.

Education: Parents and Students, please visit the “Heads Up” page from the Centers for Disease Control and Prevention (CDC) and take this FREE online course entitled “Concussion in Sports”.
